Galantamine

In 1960, galantamine-producing species were identified that are easier to cultivate and the full chemical synthesis pathway was published. This was a biomimetic laboratory process with a yield of below 1 per cent ; , which had been designed as proof of structure rather than for industrial production. Detailed research in the West only started in the 1980s when the new indication for Alzheimer's disease ; became the central focus of preclinical and clinical work. However, the use of snowdrop, snowflake, and daffodil as the only source of galantamine was wholly unsustainable. After its initial launch in western countries, galantamine remained as a critical resource. It was only available at prices around $40, 000 per kg until Sanochemia Pharmazeutika, a company based in Austria, developed a method to synthetically and commercially produce the compound in 1997. Later, galantamine was co-developed by Shire Pharmaceuticals and the Janssen Research Foundation for Alzheimer's disease. No treatment can stop Alzheimer's disease. However, for some people in the early and middle stages of the disease, the drugs tacrine Cognex ; , donepezil Aricept ; , rivastigmine Exelon ; or galantamine Reminyl ; may, for a time, help prevent symptoms from becoming worse. Some medicines may also help control certain behavioral symptoms of Alzheimer's disease, such as sleeplessness, agitation, wandering, anxiety and depression. Treating these symptoms can make patients more comfortable and ease the job for their caregivers. In the past, it was difficult for doctors to tell if a person had Alzheimer's disease. Today, there are tools that make the disease much easier to diagnose. There have been encouraging studies showing that early diagnosis and treatment can help control the symptoms of Alzheimer's disease. But early detection is the key. And knowing what to look for is an important way that you can help yourself.

Proc natl acad sci usa 2001 feb 13; 98 4 ; : 2089-9 farlow pharmacokinetic profiles of current therapies for alzheimer's disease: implications for switching to galantamine. Calomel electrode SCE ; was used as the reference electrode. An electrolyte solution of 0.5 M H2 SO4 consisting of aniline 0.5 M ; was taken inside the cell and the SS electrode was cycled between -0.2 and 1.2 V versus SCE at a sweep rate of 100 mV s-1 . A film of PANI forms on the SS electrode, and its thickness increases by repeating the potential cycles several times. The PANI SS electrode was washed copiously with 0.5 M H2 SO4 and stored in the same solution for further experiments. The PANI SS electrode was used for single electrode studies, whereas the chemically precipitated PANI was used for fuel cell studies. A laboratory scale DAAFC was assembled using carbon block current collectors and a membrane electrode assembly MEA ; . For preparation of the anode, PANI 90 wt.% ; and Nafion suspension 10 wt.% ; were taken in isopropyl alcohol and subjected to sonication for intimate mixing. The suspension was brushed on a non-wetable Toray carbon paper TGPH 090 ; of size 4 cm 4 cm. The solvent was allowed to evaporate and dried at open air. Several such electrodes were prepared by varying the loading of PANI between 15 and 60 mg. Using a similar procedure, Pt coated cathodes were prepared. A film of Nafion 117 was hot pressed at about 125 C with a cathode and a dummy carbon paper anode, which was subsequently replaced by a PANI coated carbon paper. Hot pressing with the PANI anode was avoided since PANI loses its electrochemical activity towards the oxidation of AA due to thermal degradation. The MEA was in turn sandwiched between two corrugated carbon blocks, which had provision to circulate the anolyte solution at the anode and to pass oxygen gas at the cathode. Cyclic voltammetry and chronopotentiometry of PANI SS was carried out using a EG&G potentiostat galvanostat model Versastat. Currentvoltage measurements of fuel cells were performed using a home-made galvanostatic circuit. IR spectra were recorded by a Perkin-Elmer FT-IR 1000 spectrometer and glibenclamide. Animals- Transtracheal stimulation of the spinal cord was carried out in 14 healthy research mixed-breed dogs 7 males and 7 females ; that weighed 14 to 22 and were 1 to 2 years old were examined under veterinary surveillance. The study was approved by the Regional Scientific Ethical Council for the protection of animal rights. Dogs were anesthetized by IM administration of xylazine hydrochloride 1 mg kg ; and ketamine hydrochloride 10 mg kg ; . After the insertion of a venous cannula, the dogs received atracurium besylate IV 0.1 mg kg ; for relaxation and were intubated with tubes intratracheal tube, Pulmonx, Palo Alto, California ; . Dogs were artificially ventilated with a 2: 1 mixture of air and oxygen. Atracurium besylate was maintained by 0. 2 mg kg h. This dose of relaxant resulted in partial neuromuscular blockade, yet enabled muscle action potentials to be recorded. The right femoral nerve and vein were exposed and the latter was cannulated. During the entire experiment rectal temperatures were measured and maintained within the range of 37 to degree, using a heating pad and external radiator. An ECG, arterial blood pressure, pulse, and blood gases were continuously monitored. On completing the experiments, anaesthesia was suspended by IV administration of atropine hydrochloride 0. 25 mg ; , neostigmine methylsulfate 0. 5 mg ; , and galantamine 2. 5 mg ; . After return of spontaneous breathing, tubes were removed.

Right 5 1 ; Explanation about medical care Mrs C, who was elderly, was admitted to the rest home on 15 August 2000 to be cared for while her daughter, Ms B, was overseas. Ms B filled in the admission form and attended to all the arrangements on behalf of her mother. She filled in the name of Dr D Mrs C's general practitioner and signed the admission document which states that there is a "choice of being attended by the medical practitioner of the house, or [retaining] his or her own GP". Ms B understood that her mother was to be cared for, and thus regularly reviewed as appropriate for a level three support needs patient, by the doctor contracted to the rest home, as this had been the understanding with another rest home where her mother had stayed the previous winter. Ms B understood that the name of her mother's general practitioner was obtained only so that Dr F could contact Dr D if necessary. Ms E, the nurse who admitted Mrs C, stated that it was her usual practice to advise patients and their family of the choice of medical care provision. She also stated that it was usual to explain that if the patient's own doctor was retained he or she would have to be available for house calls on a 24-hour basis. Ms E further stated that because this explanation was given, the question who would medically review the patient did not normally arise. Ms B later advised that she did not receive such an explanation. However, Ms B signed the admission document with the statement about the choice of medical practitioner. I satisfied that some explanation was given. The understanding of the rest home was that Mrs C's general practitioner, Dr D, was responsible for her medical care. Consistent with this understanding, in September 2000 when a urinary tract infection was detected, Dr D was contacted by the rest home staff, and prescribed medication that successfully treated the condition. Clearly there were different understandings about who was to provide Mrs C with medical care. I acknowledge Ms B's explanation that her understanding arose from her previous experience with another rest home and that she understood she was asked for Dr D's name only as a source of information for Dr F if required. Nevertheless, I accept that the rest home took reasonable steps to outline the options for medical care in the admission document. In addition, the explanation by the admitting nurse of the 24-hour on-call requirement if medical care by the patient's general practitioner was elected, provided an opportunity to clarify the arrangements. These arrangements operated successfully in September 2000 when Mrs C's condition was relayed to Dr D, who prescribed treatment. In my opinion, therefore, the rest home took reasonable steps to effectively communicate arrangements about medical care with Ms B and did not breach Right 5 1 ; of the Code.
