If you live in California, you may soon breathe a little easier than the rest of us, now that the state has become the first to classify secondhand tobacco smoke as a toxic air pollutant. In a unanimous vote in January, the state Air Resources Board put secondhand smoke in the same category as diesel exhaust and arsenic, citing a report published last fall that found a sharply higher risk of breast cancer in young women passively exposed to the fumes, and linked it to other cancers, asthma, heart disease and health problems in children. The immediate effect of the board's vote will be an investigation into the places where Californians most often encounter secondhand smoke and how to eliminate them, for instance, prescribing information.

Travel to developing countries is associated with an increased risk of diarrheal illness, generally caused by ingestion of enteric pathogens. Approaches to the control of traveler's diarrhea have included patient education, chemoprophylaxis, and various forms of treatment. Travelers appear to have trouble controlling dietary risks despite education 10 ; . Chemoprophylaxis with antimicrobial agents is controversial 2, 9 ; . Prophylaxis with bismuth subsalicylate tablets confers at best 65% protection 3, 13 ; . Treatments for traveler's diarrhea include fluid replacement; symptomatic relief with agents such as bismuth subsalicylate, adsorbents, and antimotility agents; and the use of antimicrobial agents as single doses or for as long as 5 days and bicalutamide.

Afflicting cloven-hoofed animals like goats, sheep and cattle, Foot and Mouth Disease FMD ; is among the most contagious agents on the planet. The virus -- which causes salivation, lameness, blisters and a severe diminution in meat and milk production -- travels up to 40 miles on the wind. It typically doesn't infect people but may hitch a five-day ride in human respiratory tracts as it seeks another host. FMD is incurable and devastates livestock health and profitability. Even animals that recover or are vaccinated act as infectious carriers. This spring, the United Kingdom's Ministry of Agriculture, Fisheries and Food ordered more than a million animals slaughtered in an attempt to stem this fearsome epidemic. Even with those efforts, FMD spread across Europe and into other nations, prompting government officials around the world to call on Antec International, a world leader in biosecurity, for help. Based in Suffolk, England, Antec pioneered the concept of biosecurity, which removes threatening viruses and bacteria from an animal's environment and then maintains the lowest possible level of disease challenge. The company assisted in previous significant outbreaks of FMD in Asia and England. "We blend a range of products and value-added services into broad biosecurity programs, " says Mark Blackwell, Antec's director of marketing and international sales, animal health division. "A huge market for our expertise exists for several reasons. First, animals are raised intensively now, which makes it easier for pathogens to spread. Second, consumers are concerned about unwanted antibiotics or food-borne pathogens in their food. By reducing bad organisms in an animal's environment, we help make antibiotic use more responsible, appropriate and specific. We also aid in reducing food-borne diseases.

Sheldon J. Segal and Luigi Mastroianni, Jr. Hormone Use in Menopause and Male Andropause New York: Oxford University Press, 2003 Hormone replacement therapy HRT ; involving combinations of estrogen and progestin for the treatment of symptoms of menopause has been used for millions of women for over 50 years. Hundreds of scientific studies have confirmed the beneficial effects of hormone therapy HT ; . So, it was with much consternation and confusion that a report was greeted in mid-2002 by the National Institutes of Health NIH ; that it was abruptly ending the first of two large studies begun in 1993 ; involving 16, 000 post-menopausal women because of little apparent benefit from HT but, on the other hand, showing an increase in breast cancer and heart disease as compared with placebo treatment. The study, part of an NIH Women's Health Initiative WHI ; created panic and anxiety amongst medical practitioners and patients. The situation has been exacerbated further by the announcement in February 2004 that the second WHI study of estrogen-only treatment of 11, 000 postmenopausal women was stopped prematurely because an interim analysis detected little benefit but some risks. Dr Lori Mosca, Director of Preventive Cardiology at New York Presbyterian Hospital was widely quoted as saying, "This is the final nail in the coffin".for HRT. Fortunately, before the completion of their book, Segal and Mastroianni were able to review the protocol and the results of the first WHI study. It is clear from their analysis that the declaration of the end of HRT is itself premature. Of the many results that they examine in detail, none support the conclusions that were widely publicized. One example concerns the protocol exclusion criteria for study participation that resulted in a ".clinical trial [that] never really represented the population for whom hormone therapy makes most sense". Hopefully, this analysis will help professionals and lay people to deal with the conclusions of the second WHI study when its details become available. The authors calm and systematic treatment of the WHI study should go a long way to calming the mediainduced hysteria and misunderstanding about the appropriate use of hormones for menopause andropause symptoms. This small 197 pp ; book is packed with the wisdom of two experienced professionals. The chapters are organized around questions that are frequently asked by physicians and patients. Menopause and andropause are treated with equal consideration. Where appropriate, scientific mechanisms and molecular interactions are clearly presented without resorting to jargon. The relation of HT to various ailments and conditions afflicting aging male and female patients is considered and evidenced-based advice is given with clear cautionary admonitions. The benefits as well as the unfounded hype of herbal and phytosteroids are treated with balance. This book is an important contribution to health care that every person approaching menopause andropause should read. Paul H. Silverman, Ph.D, D . University of California, Irvine.

Q UA L However, data must be analyzed to determine whether the problem is failing to achieve a blood pressure goal due to inadequate therapy or patient nonadherence to a prescribed regimen. Poor adherence resulting from adverse drug reactions cannot be ignored. Thus, data must be accurately recorded to determine specific problems associated with lack of control. In a study published in the New England Journal of Medicine, 18 percent of patients reported adverse effects from medications prescribed for them, yet only 3 percent of medical records indicated these events Ghandi 2000 ; . Physicians also can have a positive influence through their own initiatives. While the NCQA hypertension measure will not be changed in the near future, NCQA has initiated a program with the American Heart Association to recognize physicians who achieve blood pressure control, as well as other important quality indicators, such as LDL cholesterol goals in patients with cardiovascular disease. In this program, physicians or MCOs that wish to recognize their physicians ; can voluntarily submit data related to the treatment of their cardiovascular patients. Such reward-and-recognition programs improve physicians' awareness of processes that result in improved clinical outcomes, as evidenced by a previous NCQA partnership with the American Diabetes Association; here, physicians far exceeded national averages on NCQA diabetes measures NCQA 2002 ; . Pharmacists can support efforts to achieve better hypertension control. They are often familiar with methods used to obtain HEDIS data and can assist with efforts to acquire claims data and drug costs. Pharmacy systems may allow for access to information about patients' acquisition of drugs, missed refills, and support for physicians to monitor side effects that may result from medications. In addition, drug-utilization tracking systems support pharmacists who work with physicians on drug simplification or help to identify patients who may need assistance in following a therapeutic regimen. By educating and counseling patients when dispensing drugs, community pharmacists can play an important role. Their capability to offer information about blood pressure control, urging compliance to medications, and reporting of side effects suggest additional ways to help the patient manage hypertension. care delivery systems, and accreditation groups -- are committed to managing the problem aggressively. Many others have dedicated enormous time and resources to analyzing current management theories and best practices to determine the best way of doing so. Health plans and many providers continue to play a leadership role in this effort, guided by tools developed by NCQA and other institutions. The result has been significant gains in controlling blood pressure, along with more widespread recognition of the importance of control. Adequate control of blood pressure through achievement of goals is not easy. JNC-7 challenges health plans and professionals to improve treatment of a complex condition. The challenge is now, for example, urecholine.
One of the priorities of the MI is to find an efficient solution to the issue of prostitution. Seen in this light, the MI has drafted a Bill Regulating Prostitution. Tabled to the Government in April 2005, the bill is expected to come into effect on the date of the denunciation of the Treaty on the Suppression and Eradication of Trafficking in Human Beings and Use of Prostitution of Other Persons. Far-reaching changes have been brought about by Act No. 218 2003 Coll., on the Judiciary for Juveniles, which introduces - as of 1 January 2004 - into the Code of Criminal Procedure a new criminal offence in the provisions of 217a - Soliciting to Sexual Intercourse. This provision makes commercial sexual abuse of persons aged 15-18 years a criminal offence. At the end of 2003, the MI and the Office of the Government, acting in co-operation with the Cheb District Police Directorate and the Municipal Council in Cheb, initiated a pilot project to obtain information connected with commercial sexual abuse of children in the Cheb region and to take appropriate measures. Policemen focused on other risk phenomena afflicting the socially excluded communities. Besides the criminal sanctioning of offenders, an important role is played in this respect by contacts with the social workers of the Municipal Council and with elementary schools in the town. Such a system of co-operation is known to facilitate more efficient measures to provide assistance to child victims, including a chance to improve the social situation of their families. Part of the activities evolved and bicalutamide.

TB is spread from person to person in the air. You caught TB from someone else who was infectious with TB. You can infect other people with the TB germ, if you have a lot of TB germs in your sputum. Most TB is spread from person to person before a diagnosis is made. As soon as you start to take the medication, you will become less likely to spread TB to other people. If there are no TB germs in your sputum, or if you have TB only in your lymph glands, then you cannot infect other people. Your doctor will tell you if you are at danger of infecting other people with TB. You can reduce the risk of infecting other people by doing the following: 1. Take your medication correctly every day, even if you feel well. 2. Cover your mouth and nose with a tissue when you cough. Put the tissue carefully in a dustbin or flush it down a toilet, then wash your hands. 3. You do not need to have separate dishes, cups or cutlery. Does my family have to be tested for TB?
